甜味剂赤藓糖醇的健康效应

摘要:

赤藓糖醇作为一种天然糖醇类甜味剂,因低热量、对血糖和胰岛素影响小、无致龋齿性及高安全性等优势,在食品和保健食品等领域应用广泛,但其健康效应近年引发关注。本文综述显示,赤藓糖醇经口摄入后主要在小肠吸收,几乎不参与体内代谢,毒理学研究表明其安全性高,联合国粮农组织/世界卫生组织联合食品添加剂专家委员会(JECFA)将其每日允许摄入量(acceptalde daily intake,ADI)定为“未指定”。然而,其健康效应具复杂性:一方面,可抑制α-葡萄糖苷酶活性、降低餐后血糖波动,刺激肠道激素分泌调节饱腹感,对血管内皮有抗氧化保护作用;另一方面,血清赤藓糖醇水平与2型糖尿病、肥胖、冠心病及糖尿病肾病相关,可促进血小板凝集和血栓形成,且可能引发胃肠道反应、影响肠道菌群代谢产物、诱导巨噬细胞促炎极化,少数人群存在过敏反应。目前,尽管JECFA及多国监管机构认可赤藓糖醇的安全性,但部分研究显示其长期摄入与代谢疾病及心血管系统效应存在潜在关联。因此,仍需通过辨析外源性暴露与内源性合成的相对贡献,对其作用机制开展深入评估与探究。
